Located in Sacramento, California, Sierra Vista Hospital is a full-service mental health facility and a center of support for patients and their families. Since 1986, we have offered our clinical expertise to those suffering from emotional, behavioral and addiction problems. Our private, 120-bed facility is home to a clinical team comprised of licensed professionals committed to providing the highest quality care.
We are currently hiring a Registered Nurse. This position will prescribe, coordinate, and evaluate patient care through collaborative efforts with health team members in accordance with the nursing process and the standards of care and practices.

Complete a comprehensive nursing assessment and patient interview according to policy within eight (8) hours of patient s admission to the unit

Determines the need for further assessment/ consultation and notifies appropriate personnel.

Documents and reassesses any significant changes in the patients condition in compliance with facility s policy.

Documentation reflects plan of care and is accurate, complete and timely, following hospital policy.

Delegates staff assignments based on assessment of patient needs, acuity and staff experience.

Assesses the milieu and determines staffing needs for each shift

Demonstrates an ability to respond and provide direction with increased flexibility during crisis and emergency situations.

Completes an assessment of patient s medical condition and performs re-assessment when indicated.


Employ principles of communication, interviewing techniques, problem solving and crisis intervention when performing psychotherapeutic interventions and role models for staff.

Demonstrates Interventions are based on knowledge of psychiatric and medical nursing procedures, as well as the plan of care

Accurately administers medication to patients in accordance with hospital policy and physician orders.

Transcribe and carry out Patient Treatment Orders as prescribed and per policy and procedure.

Utilize appropriate interventions and notification in psychiatric and medical emergencies.

Demonstrate clinical competence in primary patient relationship, working with groups and families.

Notifies the physician when indicated.

Treatment Planning

Collaborates in prioritizing and formulating a plan of care based on integrated assessment.

Collaborates in developing a plan of care with specific and measurable goals, objectives and interventions, defining actions unique to each patient s needs.

Collaborates in updating and revising the plan as goals/objectives are met or when the patients condition changes, and at least every seven days.

Collaborates in documenting the treatment plan, nursing interventions and the patient s response to the intervention.


Monitor unit activity and encourages patient participation, using activities of daily living in a goal directed way to foster patient self-care.

Coordinates nursing care for patients during hospitalization.

Provide structure and maintain a therapeutic environment in collaboration with the patient and to other health-care providers.

Consistently utilize universal precautions and environmental safety guidelines to maintain an environment conducive to safety and that promotes health and reinforces the safety and dignity of the patient.

Conduct structured didactic groups with patients and / or families.

Ensures Individualized treatment plans, special programs and general milieu programming are implemented.


Provide education to patients and families based on their identified needs and limitations.

Provide and document discharge instructions including food and drug interactions, diets and activities as appropriate.

Evaluates and documents the effectiveness of all patient teaching.

Job Requirements:
Education: Graduate from an accredited school of nursing required; Bachelor's Degree preferred.
Experience: Experience in a psychiatric health-care facility preferred.
Licensure: Currently licensed to practice by the State Board of Nursing. Valid driver's license preferred.
Additional Requirements: Must pass a medication administration exam before administering medications. CPR certification, and successful completion of the hospital s designated behavioral and physical management training program including de-escalation and appropriate use of seclusion and restraint required prior to assisting in the procedures. May be required to work overtime and flexible hours.
