Job DescriptionOur Adult Day Health Care Center is looking to hire Activity Coordinator.BASIC FUNCTION:Under the direct supervision of the Program Director, is responsible for developing and implementing an activity program that meets the needs and interests of each participant. The activity program shall assist the participant in resuming self-care and normal activities to prevent further mental or physical deterioration.JOB RESPONSIBILITIES:1. Participate in initial and continuing assessment of participant needs for the development of the individual plan of care by the multidisciplinary team. All participants must be reassessed every six months.2. Maintain a signed quarterly progress record and individualized activity plan in the participant's health record.3. Develop a quarterly activity calendar prior to the start of each new quarter. The calendar shall include all monthly activities such as crafts, holidays, religious services and outings.4. Post a monthly schedule of planned activities in a central location readily accessible to participants, family, guardian or authorized representative.5. Involve participants in the planning of their program.6. Utilize volunteers as needed to implement the plan.7. Utilize center transportation to provide activities in the community as indicated by participant needs, interest and plan of care.8. Provide or supervise the activities specified in the participants activity plan with the assistance of the Program Aides.9. Order and maintain activity equipment and supplies. Provides yearly list of inventory to Program Director by June 30th of each year.10. Coordinate participant services with therapists and consultants.11. Other duties as assigned by Program Director.QUALIFICATIONS:FOR ACTIVITIES ASSISTANT POSITION NO CERTIFICATION IS REQUIRED BUT CANDIDATES WITH EXPERIENCE WILL BE PREFERRED.Must meet one of the following:1. Have two years of experience in a social, recreational or educational program within the past five years, one year of which was full-time employment in a patient activities program in a healthcare setting, mental health setting or setting for the care of the handicapped.2. Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapy assistant or a qualified social worker.3. Have completed at least 36 hours of training in a course designed specifically for this position and approved by the Department.4. Have and maintain current First Aid and cardiopulmonary resuscitation (CPR) certification within the first six months of employment.5. Seniors in adult day services programs may be physically, mentally,or functionally impaired, or they may simply need companionship or supervision during part of the day. The program is structured and designed, through a personal care plan, to cover all the daily individual needs of each participant.
